    Rates

    Generally, a New Patient Evaluation will cost between $235.00 and $275.00 depending on the services provided. On-going follow-up visits are typically $145.00. If outside laboratory tests are necessary, those tests will be billed by the laboratory and are separate from our office visit charges.

    Insurance

    We are considered “Out of Network” with commercial insurance companies. As a courtesy and at your request, we can provide a Superbill/Receipt for your visit which you may submit to your commercial insurance company for possible reimbursement, if you have “Out of Network” benefits. This does NOT apply to any Medicaid or Medicare Insurance.

    I’d recommend asking these questions to your Commercial insurance provider to help determine your benefits:

    • Does my health insurance plan include Out of Network benefits?
    • Do I have a deductible? If so, what is it and have I met it yet?
    • What is the process for getting reimbursed by the insurance company, if applicable?

    Payment

    Payment in full is due at the time of service. We accept cash, check and all major credit cards as forms of payment. We also accept payment directly on our site via this page. 

    Cancellation Policy

    If you are unable to attend an appointment, please make sure you cancel at least 24 hours beforehand. Otherwise, you may be charged $50.00 cancellation fee.
